About Grace Medical Center

Grace Medical Center, formerly Bon Secours Hospital, has provided healthcare services to the West Baltimore community for more than 100 years. As part of LifeBridge Health’s commitment to this mission, we continue to provide the highest level of care regardless of socioeconomic status or ability to pay.

Grace Medical Center was acquired by LifeBridge Health in 2019. Since then, the freestanding medical facility has become a one-stop-shop for primary and specialty care. Our team serves patients of all ages by removing barriers to care, offering personalized health plans and encouraging health literacy.


Primary Care

The Family Health and Wellness Center, located on the third floor of Grace Medical Center, is fully equipped to treat common illnesses, address minor injuries and perform routine health exams either in-person or online. Primary care pediatric services are also offered at this location and throughout the community via mobile van.

Our on-site care coordination team ensures the convenience and consistency of all care. Patients and providers have easy access to this team, which offers support services such as:

  • Patient advocacy and education

  • Personalized care plans

  • Appointment scheduling

  • Transportation to and from appointments

  • Referrals and access to additional care

  • Quality assurance

When necessary, primary care providers at Grace Medical Center can refer patients to specialists based just down the hall. Our newly renovated Specialty Clinic, also located on the center’s third floor, houses experts in cardiology, pulmonology, gastroenterology, orthopedics and more.
